Trenor's quote is really important. You have written about the bond between a parent and child (here and here for example) and I want to follow up on that. Caregivers and parents know things about their loved one that a doctor does not know — how can systems support the exchange of that knowledge?

I agree with you that tomorrow's electronic health record systems should be able to aggregate a patient's experience for the purpose of relationship enhancement.

I watch so many physician visits where there are literally seconds available for a doctor to learn who their patient "is" – yet I think this is one of the most important drivers and interests of doctors – they want to know who the person is that they are helping.

It would be interesting to take a group of patients who blog about their health see what happened if doctors had access to this information before a visit, and compare it to a visit where they did not have access to that information. I think the visit would be different, more meaningful.

Secure e-mail is a step in that direction, who will write the next generation EHR that will import a patient's RSS feed for use in the clinical encounter (with patient's permission of course),
