		<description>The Healthcare Intelligence Network is following the emergence and adoption of the medical home model at the Medical Home Monitor(http://www.hin.com/medicalhome/medicalhome.html).  We recently hosted a webinar on pilots in the diabetes medical home model.  The success of these pilots can be greatly attributed to the patients' cooperation.  Much has been written about physician's responsibilities in this emerging model, so it is interesting to hear about the patient and family's role here.  Self-management and an active role in the care plan will certainly improve outcomes.</description>
